Why dentists lose new patients before the first appointment

A dental practice is one of the few businesses where the people best placed to answer the phone are the people least able to reach it. Everyone qualified is chairside, gloved, and mid-procedure. The enquiry that arrived at ten past two does not get read at ten past two, and often does not get read that day.

What makes this expensive is not the delay itself. It is what the enquiry usually contains.

The questions are almost never clinical

Read a week of new-patient enquiries and a pattern appears immediately. Are you taking new patients. Do you take my insurance. How much is a check-up and clean. Do you have anything after work or on a Saturday. Where do I park. Can I bring my children to the same appointment.

None of those needs a dentist. Every one of them is answerable from the practice's own policies and diary, and every one of them is sitting in a queue behind someone who is busy doing dentistry. The practice has organised itself so that its most administrative work waits on its most clinical people.

  • The enquiry arrives during surgery hours, which is exactly when nobody can answer it
  • It gets read at the end of the day, or the next morning, by which point the sender has usually asked somewhere else
  • The genuinely clinical questions — the ones that really do need a dentist — are stuck in the same queue as the parking question
  • Nothing counts the enquiries that never became patients, so the practice sees a full diary and concludes nothing is wrong

Registration is a decision with a long tail

The arithmetic here is unusual, and it is worth being explicit that this is arithmetic rather than a measured figure. A patient who registers does not represent one appointment. They represent a check-up roughly twice a year, for as long as they stay, plus whatever treatment they need along the way, plus — quite often — the rest of their household.

Set whatever numbers you like against that. The conclusion does not change: an unanswered new-patient enquiry is not a lost appointment, it is a lost relationship, and it is lost over a question about parking.

Somebody choosing a dentist is not comparing clinical outcomes. They cannot assess those. They are comparing which practice replied.

Where the line has to sit

There is an obvious temptation, once something can answer enquiries automatically, to let it answer everything. That is the wrong instinct in a dental practice, and it is worth stating plainly: nothing automated should be asking about symptoms, discussing whether a patient needs treatment, or touching a clinical record.

The value is entirely in the other pile. Answer the opening hours, the insurance, the price of a check-up, the availability, the parking — accurately, in the practice's own words, within a few minutes — and the clinical questions arrive at a human who now has time to read them properly. The point is not to replace the front desk. It is to stop the front desk being the bottleneck for questions that never needed it.
